I think the only thing you can do is just to start again. Don't give her any opportunity to have accidents inside, take her out regularly, wait till she goes, praise heaps and try to always keep her in the same room as you are in the house.

    Good luck!

Do you notice a difference in how your partner deals with the toilet training compared to you? I don't think it's a matter of the dog thinking it is ok to go inside - it is very hard to convey to a dog that is not ok anyway - but not feeling motivated enough to go outside. Maybe your partner doesn't praise him enough in that situation? Or doesn't take him outside enough?

no he does the same as me ... its quite strange we take her out all the time latley since she has started going inside again (maybe every 25, 30mins) she doesnt go then as soon as she is inside again then she goes, mighty fustrating

How long do you stay outside with her? Maybe for the next little while you will just have to stay out longer until she actually goes? You'll need lots of patience and time to stick to that though. But it is going to be essential to create opportunities to praise her like crazy when she does go outside and to prevent as much as possible that she does go inside. And if she does it when you're inside with her, I think keeping her near you on a lead at all times may be a good way to go. Then at least you can immediately pick her up when she squats down and carry her out.

It'll take a lot of your time at first, but I think you might be amazed at how quickly she gets it (again) with the right re-enforcement. And then you should probably stick to praising her heaps for going outside for a while to prevent her getting the wrong idea again.

Good luck! Accidents inside are so annoying...
